AUSTIN, Texas (AP) — The state’s child protection agency says it missed warning signs that a 1-year-old who died after being beaten was in a dangerous environment.

The Texas Department of Family and Protective Services issued a statement Wednesday acknowledging the failings in the case of Orion Hamilton.

The toddler died Oct. 20 in Cedar Park when Jacob Salas, who was living with the girl’s foster mother, allegedly crushed Orion’s head.

Salas was barred from being near Orion after the state found he neglected one of his own children. But Orion’s foster mother said Salas did not live in the house and the caseworker took no action.

Salas is charged with felony child abuse. His attorney did not immediately return a message.

The agency’s acknowledgment was first reported by the Austin American-Statesman.
